diabetes mellitus patients often have down-regulated blood glucose transporters in the blood brain barrier. why? Due to this, what effect could insulin therapy have on this patient?

Insulin makes sugar more "permeable" to the cell's membrane by increasing binding to insulin receptors on target cells, in this case the brain, and opening the glucose channels.
