Sudden and unexpected deaths outside a hospital setting should be investigated for many reasons. One being witnesses that are in the hospital for example doctors nurses, and the medical technology that is in the hospital. Anything can happen outside the hospital. For example what may look like a natural heart attack may actually be from a poison or as you stated drugs that triggers a heart attack, who is to say that the person bringing the victim in saying it is a so called heart attack did not actually poison the individual or possibly give them drugs to trigger it.
