Bobby complains to his doctor of nausea and chronic fatigue, which has lasted about half a year. He has lost a lot of weight, and strangely, has a tan even though he doesn’t spend much time outside. His blood pressure is very low, and he has a weak pulse. After testing, it is determined that he doesn’t have anemia, but his plasma glucose, cortisol, and Na+ are low, and his K+ is high. The doctor orders an ACTH stimulation test, in which the levels of cortisol are measured after he is given ACTH.

(a) An imbalance of which hormone produced by the adrenal gland would be the probable cause of Bobby’s low Na+ and high K+ levels? Are the hormone levels too high or too low?

(b) Why did the doctor order an ACTH stimulation test?

(c) Which gland would primarily be affected if ACTH does not cause an elevation in cortisol secretion, and what is this condition called?

(d) Which gland would primarily be affected if ACTH does cause an increase in cortisol secretion?

(e)  Based on the fact that Bobby has a tan, which condition do you think it most likely that he has?  Why?
