Two methods for determining fat patterning are the waist-to-hip ratio and the waist circumference. The waist-to-hip ratio is determined by measuring the waist circumference at the level of the natural waist (or the narrowest part of the torso as observed from the front). The hip circumference is measured at the maximal circumference (includes the maximal width of the buttocks as observed from the side). The waist value is divided by the hip value.

If a man's waist-to-hip ratio is higher than 1.00 or a woman's is higher than 0.80, then they are considered to have a higher risk for chronic disease. For the waist circumference, a man's risk is increased above 102 cm (40 in.), and a woman's risk is increased above 88 cm (35 in.).

Recent Canadian research suggests that waist circumference is more useful in predicting obesity-related health risk than BMI.
