The sign or symptom that suggests that a patient with impaired skin integrity is developing a systemic infection is:
1.   a lesion on the patient’s leg that is swollen and warm to the touch.
2.   a temperature that has risen to 101º F.
3.   blood pressure that has risen from 126/84 to 130/86.
4.   a request by the patient for medication for severe itching.

The answer is 2. Number 3 would be more for an indication of pain, also it doesn't rise enough. Number 4 is a symptom and does not mean the client has gone systemic. Number one does indicate an infection but a localized one. Number 2 is indicating that the body is fighting an infection with the raised body temperature and is a better indicator that the infection has gone systemic.
