Initiation of drug therapy for patients with chronic obstructive pulmonary disease (COPD) is based upon determination of COPD grades or categories related to airflow limitation.  For a patient with low risk & less symptoms, the text indicates the first choice as needed medication should be:

a.) Only LABA

b. SABA or SAMA

c. LAMA or LABA

d.Only LAMA

I believe the answer is (C), although there's no way to verify since I do not know what book you're using. In a real-world clinical practice setting of COPD treatment, combined LABA-LAMA inhalers appear to be effective.
