Sonata® (Zaleplon), a non-benzodiazepine hypnotic, is used for the short-term treatment of
insomnia. The normaldose of Sonata® is 10 mg/day. From literature studies, it is known that
10% of Sonata® is excreted unchanged in the urine. What will be the new adjusted dose of
Sonata® for a female (55 kg) with creatinine clearance of 85 ml/min?

Out of the 10 mg/day, 10% is excreted, which makes up 10(0.1) = 1 mg/day.

Adjusted dose = Normal dose x (1 - % excretion) x (1 + renal adjustment factor)

The renal adjustment factor is calculated based on the patient's creatinine clearance (CrCl), which reflects the patient's kidney function. The formula for renal adjustment factor is:

Renal adjustment factor = 0.72 x (CrCl/50)

Using the above formulas, we can calculate the adjusted dose of Sonata® as follows:

Renal adjustment factor = 0.72 x (85/50) = 1.224
