  to give guidance to the person who has durable power of attorney.
  2.
  to give caregivers authority to allow a patient to die with dignity.
  3.
  to allow an individual to instruct caregivers prior to surgery what their wishes are about end-of-life decisions.

The purpose of advance directives is to give the person an opportunity to make decisions regarding healthcare before an illness or a need for treatment that would prohibit making such critical decisions. The major decision made is in regard to resuscitation. Some people have assigned a durable power of attorney to someone they trust to make healthcare decisions in their stead. Often, people designate on their advance directive a person assigned to sign consent forms and make other decisions about the welfare and treatment of the patient.
