Serum protein electrophoresis on an elderly patient reveals a monoclonal protein spike. Increased plasma cells are present in the bone marrow.
 
  They are CD19 and CD20 negative but demonstrate high-density CD38 and monoclonal cytoplasmic immunoglobulin. What is the most likely diagnosis?
