1.   DIAGNOSIS:  Scheduled second cesarean delivery due to previous low transverse cesarean delivery, 38 weeks, single liveborn, no complication (code diagnosis codes only) 

2.   Full term infant, born in hospital, vaginal delivery. Meconium staining was noted at the time of delivery. There were no complications in the infant as a result of it.

3.   Newborn male, born outside of hospital (in private care while being driven to hospital), vaginal delivery
