1.   Admission for chemotherapy for patient with metastasis to bone; patient has a history of breast carcinoma with mastectomy performed eight years ago.

2.   Emergency department visit for a patient who needs replacement of a clogged gastrostomy tube.

3. The patient is a 30-year old female who is in her 22nd week of her first pregnancy and is seen in the obstetrician’s office for her monthly evaluation.  Nor problems were identified, and the patient will be seen in four weeks.  The diagnosis for this visit is first pregnancy, normal supervision visit. 

4.    Visit to physician for prescription of initial prescription of birth control pills
