What are some consequences of awaiting trial in jail versus being out on bail? Are those consequences important enough to be concerned about the fairness of jail?

Those are very serious consequences and do raise concerns about fairness. Options that can be noted include increased use on non-financial alternatives and to pretrial services supervision.
