I would like to know on how body handles and remove scar tissue within body, so those area would be fully recovered. 

Furthermore, referring to following different stages, which stage is fully recoverable to remove scar?

1) mild hypoxia in the early scars
2) moderate hypoxia in the proliferative scars
3) Severe hypoxia then occurs in regressive scars,

Do you have any suggestions?

Scar tissue cannot be repaired by the body, other than to form new collagen fibers to mend the damage. Look at a superficial scar on your skin, that scar will remain there forever. Whether it be on your skin or within your blood vessels, the scar formed will have a different texture and quality than the surrounding tissue.

Scars form after a wound is completely healed. If the wound doesn't completely heal because the inflammation is chronic, fibrosis will continually occur in that area until the inflammation goes away. This is what happens with people who suffer from autoimmune diseases, leading to arthritis or psoriasis of the skin.
