Which of these questions would be most important for a nurse to ask a patient who reports a change in bowel habits?
1. “Can you describe your activities in a normal day?”
2. “Do you have rectal pain?”
3. “Have your bowel changes created stress for you?”
4. “Can you describe the consistency of your bowel movements?”

4 -- All of the questions may be utilized for data collection when a patient reports with concerns related to gastrointestinal function. Questions or statements that are open-ended will allow the patient to provide the greatest amount of information.
