Just as the question asks, do med school graduates applying for a job as a physician have to give details of their own medical history to the hospital? I would understand medical allergies but I'm wondering more along the lines of psychiatric and personal history. Are they allowed to ask for that and, if so, are they allowed to use that to base their decisions?
Lastly, are patients allowed to ask for this information?

Yes to all your question. All hospitals have to have Credentialing Committee to review all the applicant's personal, medical, psychiatric, malpractice and police records. Hospital can use these informations to accept or not accept the physician's applications.

As far as patients asking these questions - I don't think most hospitals will release them to you (confidentiality issue),  but you can get these informations in the State Registration Departments.  These may vary with different States.

The credentialing committee of any hospital has NO access to a physician's personal medical records.

There are some standard questions that we are asked when applying for pretty much anything:  Do we have a criminal record?  Do we have a drug or alcohol problem?  Do we have any medical problems that could interfere with the performance of the job, including psychiatric problems?  Have we defrauded Medicare or Medicaid?  

We answer those and sign that what we said is true.  If it is found out later that someone lied, the lying alone is usually grounds for dismissal and/or revocation of privileges.

No, they don't ask about allergies.  They don't care.

Many hospitals require annual tuberculosis testing, and proof of hepatitis immunization, as a public health measure.

BTW, few physicians are employed by hospitals outside of academic centers.  Most join groups or start their own practice (self-employment).  Some groups (like anesthesia, pathology, radiology) will contract with a hospital to provide exclusive services, but are not employed by the hospital.
