You are working in a large city hospital emergency room.  Two men have been brought in from a homeless shelter.  Both are complaining about shortness of breath, but the initial exam has ruled out heart attack.   When you examine them, you notice that one of the men has oral thrush.  Both men have a long history of alcoholism.  What does the presence of oral thrush indicate?  Why would one of the men have the condition but not the other?

What does the presence of oral thrush indicate?

It means that the person has a weakened immune system. Oral thrush occurs when a yeast infection develops on the inside of your mouth and on your tongue. Usually the immune system keeps fungus in the oral cavity in check, but when it is weakened, as a result of AIDS for example, this fungus grows uncontrollably.
