Read the following case example and answer the accompanying questions.
   Mr. Hollister has accompanied his wife to your facility, where she is to sign the consent for a colonoscopy. He states that since his wife lost her memory he usually makes the decisions for her and that he will sign the form. Mr. Hollister asks the AHP to explain the procedure including any possible complications. The AHP answers the question to the best of her ability but is not sure she included all of the required information. The AHP is now questioning if she should have allowed MR. Hollister to sign the consent.
      Was the AHP working within her scope of practice? Why, or why not? What alternative action could have been taken?

The AHP was not working within her scope of practice. She is not qualified to explain a diagnostic procedure that she does not fully understand. The primary responsibility of explaining the procedure is with the physician or his qualified delegate who is comfortable with their base of knowledge. If the wife is incompetent, the husband, as the next of kin could sign. The AHP should seek guidance to determine if Mrs. H. is indeed incompetent. An alternative action AHP could have taken was to ask Mr. Hollister to wait while you get the doctor to explain the procedure and identify the risks.
