The nurse should allow the client to make a choice about using soap on the face. Avoid using soap on the face unless the client wants it, because soap is drying, especially to the face. Ask the client to rub soap on his hands or do it for the client because soap and friction help to remove microorganisms. The washcloth should not be kept open to wipe the face; instead, it should be wrapped around the hand with the ends tucked in to form a mitt. Loose ends that remain exposed can drag over the client's face, possibly into his eye, causing discomfort.
