The text describes worse mental health among people in the lower class. Why might the relationship between mental health and social class be reciprocal?  For example, why might being mentally ill increase the likelihood of becoming lower class?

People who are mentally ill would have more difficulty maintaining a good job, a home, and supportive social relationships, so they are at risk of moving down the social class structure. The lower class also have less opportunity to pursue quality treatment from a family physician who would understand the individual's personal situation and physical health better than a public health official or emergency room physician.
