1 -- Each symptom identified should be thoroughly described in order for the nurse to gain a full understanding. The nurse may ask the patient questions such as “Tell me more about the problem,” or “How did the problem start?” These types of questions will usually prompt the patient to proceed with further information about the symptoms. Skillful interviewing techniques may gain the patient’s trust, discard unimportant data, and speed up the interview, but these are not as important as exploring and investigating symptoms.
