A nurse hears a colleague including all of the following statements when providing teaching to a patient with Cushing’s syndrome regarding increased risk for infection. Which one would require the nurse to intervene? “Cushing’s syndrome increases the risk of infection because
1. epidermal hypertrophy restricts macrophage activity.”
2. cortisol affects protein synthesis.”
3. cortisol inhibits collagen synthesis.”
4. resulting edema impairs blood flow to tissues.”

1 -- Elevated cortisol levels impair the immune response and put the patient with Cushing’s syndrome at increased risk for infection. Elevated cortisol levels result in epidermal atrophy, not hypertrophy. Macrophage activity is not a related action. Increased cortisol affects protein synthesis, causing delayed wound healing, and inhibits collagen formation, which results in epidermal atrophy, further inhibiting resistance to infection. Impaired blood flow to edematous tissue results in altered cellular nutrition, which increases the potential for infection.
