it should be firmly applied around the arm, placed about an in inch above the antecubital space (not over it) ans should cover at least 2/3s of the upper arm.

4 -- Appropriate technique for blood pressure assessment involves using a cuff that completely encircles the arm and snugly covers two-thirds of the upper arm. The bladder should be centered over the brachial artery, with the lower edge 2-3 cm above the antecubital space.
