Even though the same medication may be available in more than one form (e.g., oral or parenteral), one form should not be substituted for another if the prescribed form is not readily available. If a medication is prescribed for oral administration and the client's condition has changed, making it impossible for the client to swallow, the nurse must consult the health care provider to prescribe administration by another route.
