Documenting information on the client's appearance, behavior, and responses assists other caregivers in efficient client care. Ensuring that the client or the client's family interacts with other caregivers is not an essential requirement for the care providers to provide efficient care. Ensuring that the care providers are assigned only for night shifts may not always be feasible and so it is not an appropriate nursing intervention.
