You are working with an elderly client whose memory has become progressively worse over the past 2 years. Her husband, because of her worsening incontinence, brought this client to the geriatric clinic.

During the evaluation, you also learn that she is beginning to have difficulty finding her way home from the grocery store, which is only a few blocks away, and that she is becoming increasingly suspicious and paranoid. In helping the husband understand the paranoid behavior that his wife is exhibiting, you would offer which of the following explanations? a. The paranoia and suspiciousness are due to unresolved feelings of guilt.
  b. As memory worsens, paranoia and suspiciousness often occur.
  c. The reason for the suspiciousness should be discussed with the person with dementia.
  d. The paranoid behavior is certain to get much worse as the disease progresses.

B
Paranoia and suspiciousness often accompany worsening memory loss. For example, it is quite common for individuals suffering from such memory loss to forget where they had previously placed items, and subsequently suspect family and friends of stealing them. Such persistent paranoia can be very draining on other family members. In some cases, delusions and hallucinations may accompany the paranoid ideations.
