A client with severe orthopedic injuries after an automobile accident is irritable, angry, and belittles
  the nurses who provide his care.
 
  While the nurse is changing the dressing over a deep laceration, the
  client screams Don't touch me You are so stupid that you will make it worse The intervention
  that uses a cognitive technique is to
  a. wordlessly leave the room.
  b. continue the dressing change, saying Did you know that the dressing change is
  necessary so your leg will not become infected?
  c. stop the dressing change, saying Since you think you know more than a nurse,
  perhaps you would like to change your own dressing..
  d. continue the dressing change, saying Unfortunately, you have no choice in this
  because your doctor has ordered this dressing change..

Anger is cognitively driven. Option B is an intervention that helps the client test his cognitions and
may lead to lowering his anger. Option A is not possible if the nurse has started the dressing change.
Option C will escalate the client's anger because it is belittling. Option D will escalate the client's
anger by increasing his sense of powerlessness.
