The LPN answers the phone at a primary care physician's office. The client reports, I recently started taking hydralazine (Apresoline) and have been feeling very nauseated. Which of the following responses by the nurse is best?
 
  A. You should try taking half a pill for a few days until the doctor is able to call you back.
  B. This is a side effect of the medication, and I will leave a note for the physician to determine what course of action should be taken.
  C. Because this is common with the medication, I suggest you try eating saltine crackers frequently throughout the day.
  D. I think you should call the pharmacy and see what they suggest.
