A PA chest reveals the left sternoclavicular joint to be closer to the midline than the left. What is the positioning error?
 
 the right shoulder was positioned closer to the IR than the left
  the left shoulder was positioned closer to the IR than the right
  the patient's shoulders were not relaxed
  the shoulders were not rolled forward
