A teen that is taking antipsychotic medications is complaining to the nurse that her tongue feels very thick. The nurse assesses the teen and notes that her eyes continuously look at the ceiling, and the tongue is not swollen.
 
  The nurse knows this could be a sign of: 1. Hallucinations
   2. A side effect of the antipsychotic medications.
   3. A chronic delusional state.
   4. Dystonia.
