Only highly trained personnel should perform endotracheal intubation, and each attempt should not exceed 30 sec, because gas exchange is absent during the procedure.

The first and most common problem is unrecognized equipment failure.

(Answer to Q. 4)  ANS: D
Because most cases of cardiac arrest are caused by arrhythmias, electrocardiographic monitoring should be started as soon as the necessary equipment and personnel arrive.
