Answer to #1

ANS: D
One of the most important components of the culture change movement is the creation of models of care that value and honor the important work of nursing assistants. Until our society makes a real commitment to providing adequate wages, individual supports (e.g., health insurance, education, career ladders), and an appreciation of their significant contribution to quality of nursing home care, these neglected workers cannot be expected to have the energy or incentive to extend themselves to the elders in their care. The remaining statements, while true, lack the focus on the value of the work CNAs perform.

Answer to #2

ANS: C
The role of the nurse is to assess all aspects of the resident's health and wellness. As a facili-tator of a milieu there is an expectation that the assessment would be completed and the nurse manager has a responsibility to both the nurse and the client to discuss why the nurse failed to do so. Returning the form with a discussion first will have little impact on resolving the issue in the future. While it is best to perform the assessment when the resident is com-fortable there is no reason to postpone the assessment. Asking another nurse to complete the assessment will not allow for the situation to be discussed and resolved.

Answer to #3

ANS: C
A variation of multigenerational housing has long existed in what has become known as granny flats. These may be apartments added to existing homes or the construction of small housing units on family property with privacy as well as sharing of time and resources. Such arrangements allow families to be close enough to be of assistance if needed but to remain separate. A room within the home may not allow the family member the personal freedom they want and are capable of while the care provided by the residential care facility may not be required by the individual. A house may be too much of a commitment for this individual.

Answer to #4

ANS: A, B, C, D
The correct options are reflective of symptoms of potential coronary heart disease; poor concentration, change in personality, changes in bowel habits, and disturbances in gait and balance. Weight loss rather than weight gain is seen in coronary heart disease.

Answer to #5

ANS: B
Older adults with HIV may also be at more risk for cognitive decline and may be misdiag-nosed with Alzheimer's disease (AD) instead of AIDS - the actual problem. Homan's sign is an indicator of deep vein thrombosis; Chvostek's sign is a facial muscle twitch seen with hypomagnesemia, and a neurological exam is going to identify a cognitive deficient.

Answer to #6

ANS: B
Individuals with diverticular disease need to increase fiber to 25 to 30 grams per day, and drink 6 to 8 glasses of caffeine-free fluid per day. There is no need to avoid a bedtime snack.

Answer to #7

ANS: A, B, D, E
Topics that will be included in this client's discharge teaching include pursed lip breathing, postural drainage techniques, medication education, and safe sexual activity. Diet is not generally a factor in this disease process.

Answer to #8

ANS: B
The needs of older adults for love and intimacy remain the same regardless of whether the individuals are institutionalized. Meeting the needs of the residents for sexuality and intima-cy is as important as the need for food and hydration. Sexual desire is present in older adults. Sexual activity is not dangerous for older adults. Some accommodations for chronic condi-tions might be required.
