There are actually more than 4 (which can be found in any BIology text or online) but here are 4 of them:   there must be random mating;  the population must be very large; there can be no genetic drift;  there cannot be any immigration or emigration...

As long as the five major assumptions of HW equilibrium hold:
1. No Natural Selection                                                                                          
2. Infinite Population Size                                                                                
3. No Gene Flow                                                                                                
4. Random Mating                                                                                            
5. No Mutations

Those conditions are never met in reality.  LOL.

Anyway:
1.  No selection
2.  No gene flow (no immigration or emigration)
3.  No mutation
4.  No genetic drift (which means your population has to be infinitely large and allowed to grow infinitely)

Usually a fifth is listed
5.  Random mating
but this is actually covered under the "no selection" rule since nonrandom mating would be sexual selection
