The research-based definition of a medication administration error is:
A) Any preventable event that could cause or lead to inappropriate medication use or patient harm while the medication is in the control of the healthcare professional, patient, or consumer.
B) Any deviation from the prescriber’s written order, or as entered into a computer system by the prescriber.
C) Any error related to professional practice, healthcare products, procedures, and systems, including prescribing, order communication, product labeling, packaging and nomenclature, compounding, dispensing, distribution, administration, education, monitoring, and use.
D) Any dose given that contains the wrong number of preformed dosage units (such as tablets) or was, in the judgment of the observer, more than 17% greater or less than the correct dosage.
