A client has the goal statement “Client will have clear lung sounds bilaterally within 3 days.” One intervention to meet this goal is for the nurse to teach the client to cough and deep breathe and have the client do this several times every 2 hours. At the end of the third day, the client's lungs are indeed clear. What should the nurse do to relate the intervention to the outcome?
1. Ask how many times per day the client practiced the coughing and deep breathing exercises.
2. Tell the client that the lungs are clear.
3. Document the assessment findings to show the effectiveness of the intervention.
