3,1,4,2 -- The sequencing of the assessment is important to ensure the maximum amount of information is obtained by the examiner. The nurse should look at the abdomen for symmetry, contour, and general appearance prior to touching the abdomen. Next, the abdomen should be assessed for the presence of bowel sounds. Percussion in each quadrant will follow. Palpation is the final. It might result in discomfort, and should be completed last. This will ensure the maximum amount of assessment data are obtained.
