This looks cut off...hmmm i cant figure the answer since the pulp of the info i need isnt there. Mind repasting it?

 I believe lower extremities where it is cut off is for edema. It doesnt seem complete yet still... I highly doubt its 2 or 3 from the info i have so far, its between 1 or 4

4 -- If the patient has attended a recent family or social gathering in which food was served, it is possible that the sodium content of the food was higher than the patient anticipated. The other options do not apply to help determine why the patient may suddenly be experiencing an exacerbation of the heart failure.
