A patient reports to the clinic with an open area on his penis. Which question is most important for the nurse to include in the data collection?
1. “When did you initially notice this open area?”
2. “Do you think you have a disease?”
3. “Are you promiscuous?”
4. “Have you had sexual intercourse recently?”

1 -- It will be of importance to record the onset of the open area. The remaining questions are closed, and will not elicit much information. Asking the patient about promiscuity is judgmental. Determining the date of the last episode of sexual intercourse might be indicated later if a disease is diagnosed.
