A nursing instructor is explaining the initial steps of most nursing interventions. The instructor recognizes that additional explanation is required when a student nurse states:
 
  1. You should always check the chart to be certain of a physician's or other health-care provider's order.
  2. You should always carry out the physician's order as quickly as possible without question.
  3. You should always explain the procedure to the patient using words the patient understands.
  4. You should always think critically about the order to make sure the patient's condition has not changed in such a way that the order might no longer be appropriate.
