The scenario is an example of an unmet outcome. During evaluation, the nurse is able to deter-mine that the client has not met the expected outcome of decreasing smoking by one cigarette each week, but rather has increased his smoking.
The goal may or may not have been realistic, but the nurse is evaluating whether the expected outcome was met, so this is an example of an unmet outcome.
The client is noncompliant.
The goal is measurable. During evaluation, the nurse determines whether expected outcomes are met to judge if certain goals have been met.
