The client has been told that he has a heart murmur. He asks the nurse to explain the nature of a heart murmur. What is the nurse’s best response?
A.   It is the rushing sound blood makes moving through narrow places. Some murmurs are harmless and others indicate problems.
B.   It is the sound of the heart muscle stretching in an area of weakness and usually indicates impending heart failure.
C.   All hearts have murmurs. It is just a term doctors use to describe how well the blood circulates in the heart.
D.   Heart murmurs are related to blood pressure. It is the sound the heart makes when it has to work too hard.
