If your heart is working overtime it is because there are conditions that is causing the heart to try and attempt homeostasis in the body. When you have chronic anemia, depending on which anemia it is, there is problem with your RBCs and they are lowered so your heart's output increases either to deliver more red blood cells. Or your heart is trying to deliver oxygen to the body because of the depleted levels.

2, 3, and 4 all would lower your cardiac output.

High-output heart failure occurs in patients in hypermetabolic states such as anemia, hyperthyroidism, pregnancy, and infection, which require increased cardiac output to maintain blood flow and oxygen to tissues. Hypertension is typically associated with low-output heart failure. Sedatives and analgesics slow metabolic function.
