A 40-year-old client has an episode of vomiting. On observation of the vomitus, yellowish-green color was noticed. What is the most likely cause of this finding?
 
  A) Forcing back of intestinal contents
  B) Presence of bile pigments and salts
  C) Bleeding in the lower digestive tract
  D) Bleeding inside the stomach

Vomitus that contains bile is yellowish or greenish. Presence of coffee ground-like material in vomitus is a sign of bleeding in the lower digestive tract. When the vomitus contains intestinal contents that have been forced back, there is a fecal odor, not a coffee ground-like material. In case of gastric bleeding, the vomitus may contain bright red blood.
