2. A person who was born in Puerto Rico would be categorized as Hispanic.
3. An Asian is a person who has origins among any of the original people of the Far East.
4. A person who has roots among any of the original black racial groups of Africa would be categorized as Black.
5. The patient can be categorized as being White and Hispanic. The category of other would not apply for this patient.
