The nurse should record lesions less than 0.5 cm in diameter with a well-defined border and elevated cavity filled with serous fluid as a vesicle. A pustule has a well-defined border and elevated superficial cavity filled with pus. A papule is less than 0.5 cm in diameter, palpable, and elevated solid tissue. A macule is less than 1 cm in diameter with a well-defined border, flat, nonpalpable skin surface with no change in color.
