The nurse should incorporate teaching plans, transfer notes summarizing care, and discharge summaries in the client's discharge plan when he or she is to be transferred to another healthcare facility. Doing so enables the other care providers to ensure continuity in care. The nurse need not provide referrals to community health centers or to telephone consultation services, because the client is going to be cared for by healthcare providers and has not been discharged yet. The nurse should focus on long-term goals for the client instead of short-term ones to allow the client to receive a better outcome from healthcare.
