A client has been admitted to an acute medical unit in apparent alcohol withdrawal. The client's admission orders specify the hourly use of the Clinical Institute Withdrawal Assessment of Alcohol Scale, Revised (CIWA-Ar).

During this assessment, what question will the nurse ask the client?
  A) How badly do you want a drink?
  B) Do you feel nervous right now?
  C) How long has it been since your last drink?
  D) Are you feeling angry or annoyed?

Ans: B
Feedback:
The CIWA-Ar consists of numerous questions that gauge the signs and symptoms of alcohol withdrawal. These include questions about nervousness but not questions about alcohol craving, time since the client's last drink, or anger.
