To correct an entry in a hard copy health record, you should:
 a. draw a single line through the entry being corrected, date and sign the entry.
  b. write the correct information, change the date and the time, and sign it in the side margin.
  c. remove the erroneous entry and prepare a new entry indicating the erroneous entry has been removed.
  d. erase or wite-out the entry, write the correct information, date and sign the entry.
