If a mistake is made in the computer chart documentation, the best course of action is to do which of the following?
 
 Remove the entire notation and insert a new one with the correct information.
  Make an entry on the day you notice the mistake, note the mistake and date of mistake and type the correct information in and include your name at the end.
  Tell the dentist and let him make the correction.
  Note that there is an error in the note below the original entry and type in your initial.
