IA nerve blocks have a relatively high incidence of inadequate anesthesia. Possible reasons include accessory innervation to the pulps from any of the following nerves: mylohyoid, buccal, lingual, contralateral incisive and mental nerves, sensory fibers traveling with the motor fibers of the muscles of mastication, and bifid inferior alveolar nerves or branches from the cervical plexus to the anterior teeth. Alternate injection techniques may include: mylohyoid, PDL, Gow-gates, Vazirani-Akinosi or intraosseous injections.
