A 3 month old is in the pediatric clinic for her well-child checkup. The mother states that the child's diaper area is very red and tender with some raised bumps.
 
  The doctor diagnoses the child with candida albicans and prescribes hydrocortisone for the area. The nurse is supposed to teach the mother about the signs and symptoms of possible infection and care to the area. The teaching should include all except: 1. Reporting if the sores increase in size.
   2. Reporting if the sores decrease in size.
   3. Reporting if the sores have purulent drainage
   4. The use of a mild soap to cleanse the area at each diaper change.
